Collection, Use and Disclosure of Personally Identifiable, Confidential and Sensitive Information

In order to submit an order for term life insurance, Vive necessarily involves the collection of information that identifies users and their clients and that is not generally available to the public, including without limitation names, contact information, social security numbers, drivers license numbers, etc. The sole use to which this information will be put by Vive is the processing of orders for term life insurance, including without limitation the completion, execution, and submission to the carrier of an application for life insurance and related documents requested by the user on behalf of his or her client by placing an order through Vive. By completing the online Vive order form, users authorize us to provide the submitted information to the selected insurance company and providers involved in the Vive process. This information will be used to prepare and submit the requested application for insurance. The selected insurance carrier and/or service providers may contact the user and/or the proposed insured in connection with the processing of the order placed on Vive regardless of their presence on any local or national no call list. We reserve the right to disclose personally identifiable, confidential and sensitive information collected as part of the Vive process if requested by law enforcement officials or when required or permitted by law such as when required in response to a court or administrative order.

Security

The security of personal information collected during the Vive process is important to Vive. We employ reasonable measures to protect such information during the course of the Vive process, including encrypted electronic transmissions. However, no method of storage or transmission is absolutely secure. Accordingly, while reasonably appropriate steps are taken to protect such information, we cannot guarantee its security absolutely. By using this site and the Vive process, users acknowledge, accept and agree to assume the risk of potential disclosure both on behalf of themselves and their clients.
